1. Company Registration Form
Sdn Bhd: Form 9/24/44/49/M&A with Certified True Copies
Enterprise: Full Set SSM

2. All Director NRIC

3. Bank Statement (latest 3 months)

4. iPay88 Application form & Fee Structure

5. Photo of Retail: 4 pieces (Exterior/Interior/Product)

6. Operation Hours & Write up(to intro your business)

from now onward the fee structure for Boost need amend the 0% (Promotion untill 31st December 2019)
if not all will have reject and the Application form need use the latest one ya previous version Application cant be accept anymore.
